Effective January 1, 2026, Allegiant Rx (a division of Allegiant Care) will be
partnering with MedOne for your home delivery. OptumRx will no longer be
your home delivery provider after 12/31/2025.

In mid-December, you will receive a welcome letter, new prescription ID
cards, and instructions for ordering your home delivery prescriptions through
MedOne. Begin presenting your new MedOne ID card to your retail
pharmacies after December 31, 2025.

If you currently use the home delivery through OptumRx, and you have refills
remaining at the end of the year, you do not need a new prescription, your
prescription will be transferred to MedOne. Be sure to refill any prescriptions
that are low in December through OptumRx to ensure a smooth transition
and you do not run out of medication.

If you are currently taking a specialty medication, MedOne has a dedicated
team of Patient Care Coordinators who will assist our members with specialty
medications. A Patient Care Coordinator will be reaching out to you prior to
January 01/01/2026 to identify the pharmacy where your specialty

Your pharmacy benefits are managed through a drug formulary, which is a comprehensive list of covered medications. When multiple drugs treat the same condition, some options may not be covered. Formulary updates typically occur on January 1st and July 1st. If one of your medications is scheduled to be removed in January, MedOne will notify you by mail in late November. If a medication is no longer covered, therapeutically equivalent alternatives will be provided. Please review these options with your physician before making any changes to your treatment.
